If your student received a physical for the 2019-2020 school year, they will not need one for the 2020-2021 school year. If your child’s last physical was in 2018-2019 school year, you will need to fill out the 1st page of the Health history on the UIL physical form. If there are any yes boxes checked on questions 1-6, your student must receive a new physical for the 2020-2021 school year. If there are not any yes boxes checked on questions 1-6, your child will not be required by UIL to receive a new physical. Your student must bring the new physical or the Health History form to either strength and conditioning camp June 8th (if you attend) or the first day of practice.
